Dear Patients,

Thank you for choosing Dr.Carolyn Salafia to assist your physicians in your care. Dr. Salafia is part of the New York Methodist Hospital's Outreah Laboratory. Our Pathology Department is happy to serve you in any way we can. The following information will provide you a step-by-step guide to having your consultation processed.

1. Please fill out the information request form and fax it back to 718-780-7187

2. Once we receive this information we will contact your physician and have them fax us a request for consultation

3. After we obtain the request from your physician we will let you know, so you can arrange for the slides that are being reviewed to be sent to our facility.

a. If the physician has the slides they may send them to us directly
b. If you have the slides you may send them to us directly
c. If the hospital(s) that you have had procedures at still have the slides you may request them to send the slides directly to us

4. The address to send the slide(s) to is:

5. Once we have received all of your information, your physician's consultation request, and the slides, your case will be processed.

6. Your physician will contact you once they have received our report.
