Dear Provider,

Thank you for choosing Dr. Carolyn Salafia at New York Methodist Hospital’s Outreach Laboratory to aide you in your patient’s care.  Our Pathology Department is happy to serve you in any way we can.  The following information will provide you a step-by-step guide to having your consultation processed. 
  1. Please have your patient fill out the information request form and fax it back to 718-780-7187.

  2. Please also fax to us a request for consultation with appropriate diagnostic codes and an indication as to how many slides from how many procedures we should be expecting to receive.

  3. After you fax the request please send us the slides, have the patient send us the slides or if the hospital(s) that the patient’s have had procedures at still have the slides you or the patient may request them to send the slides directly to us.

Once we have received all of the requested information, and the slides, your patient’s case will be processed. 
  1. The report will be mailed to you directly.  If you wish the report to be faxed please let us know.
